Initial Consultation and Care Planning

Our services are only provided following a referral from your physician or other licensed healthcare professional.  After your physician deems a neurologist necessary to treat your condition, their office will contact us to schedule your initial appointment. You will receive a reminder phone call from our office approximately 2 days prior to your appointment. During the consultation, we will sit down with you to review your neurological condition and the steps needed to begin care.

Should you need to reschedule that appointment,
please contact our office at least 24 hours in advance at 316-261-3220. A welcome letter will be mailed to you to complete, providing time allows. Please visit Forms & Resources to complete the forms needed for your first appointment.
